W. Va. Code § 47-8-2 says the form "shall be executed and duly acknowledged", but the Secretary of State's own Form TN-1 — the form for individuals, sole proprietors and general partnerships — has no notary block: the signer "duly acknowledges compliance" on the form itself, and it can be filed online. If you are unsure which to follow, ask the Secretary of State.
