Any Virginia local health department can issue a death certificate no matter where in Virginia the death happened, and every DMV customer service center sells them over the counter — DMV uses its own form DL 82 rather than this one. Neither is this office. Virginia charges $15.00 for each certification, and keeps it as a search fee if no record is found; a returned check costs a further $50.00. Make it payable to State Health Department and enclose an enlarged, clearly legible photocopy of your ID. Posting this takes about four weeks against roughly two weeks online.
