Send a check or money order to Tennessee Vital Records in Nashville — never cash — or take the form to ANY Tennessee county health department, which can issue any birth or death certificate registered anywhere in the state and usually does it the same day. You do not have to go to the county where the event happened, and it is not this office. This office holds births from the past 100 years only; older ones are at the Tennessee State Library and Archives. A copy of a Voluntary Acknowledgment of Paternity is $5.00 when bought alongside the certificate.
