This price is on the Department of Health's own application form (May 2025), and that form is the source: the statute names no amount — 63 O.S. § 1-325 says “The State Board of Health shall prescribe the fees”. Each additional copy is $15.00 too. The County Clerk, the Court Clerk and the county health departments do not issue death certificates.

Vendor orders cost more than $15.00. By mail, on the form below, about four weeks: Vital Records Service, Oklahoma State Department of Health, PO Box 248964, Oklahoma City, OK 73124-8964— checks payable to “OSDH VR”, no cash. The Department's lobbies are not open to walk-ins.

Who can order, and what to send

A death record is restricted to the people the form lists unless the death occurred at least 50 years ago, when it is an open record. Send a photocopy of a current, valid photo ID and any proof of eligibility the form asks for; an incomplete request is not processed.
