Set by ORC 3705.24 from 1 January 2025. Make the check or money order out to Treasurer, State of Ohio and post it to Vital Statistics, PO Box 15098, Columbus. Not to this office. The state holds death records only from 1 January 1972 — for a death before then this form is no use, and you should write to the Probate Court in the county where the death happened. Local health departments also issue same-day copies at their own fees.
