If you need a certified copy of a current death certificate, that is not this office. It comes from the Missouri Department of Health and Senior Services, Bureau of Vital Records — the route above.
One form covers birth, death, fetal death and stillbirth — tick the record you need. A death copy is $14.00 for the first and $11.00 for each additional copy — note this differs from the birth fee on the same form. Mail-in requests must be notarized and include a self-addressed stamped envelope — an un-notarized one is rejected. Going in person needs photo ID instead, and any of Missouri's 115 local public health agencies offers walk-in service; otherwise mail it to the Bureau of Vital Records in Jefferson City. Not to this office. State records begin 1 January 1910.
