Apply for a Passport in Ottawa County, MI
Here's what to expect at the County Clerk — it is on the U.S. Department of State's list of passport acceptance facilities.
Federal fees, the same everywhere (22 CFR 22.1): a $35 execution fee paid to the acceptance facility, and for an adult passport book a $50 application fee plus the $80 security surcharge paid to the U.S. Department of State. First-time applicants use form DS-11 and apply in person.

What to bring
- A completed DS-11, UNSIGNED. You sign it in front of the acceptance agent — a form signed at home has to be redone.
- Proof of citizenship: a certified birth certificate with a raised, embossed or multicoloured seal, or a prior US passport. A hospital souvenir certificate is not accepted. If you were born in Michigan, the county clerk on this page can usually issue that birth certificate.
- A photocopy of the front and back of your ID on plain white paper. Most acceptance facilities will not make it for you.
- Two payments, not one: a check or money order payable to "U.S. Department of State" for $130, and the $35 execution fee to the acceptance facility, which many Michigan counties take separately and in different forms.
- One 2x2 inch color photo taken within the last six months. Several Michigan clerks take passport photos on site for around $10.00.
Everyone applying for a first passport must appear in person, children included — and both parents or guardians must appear for an applicant under 16, or the one appearing brings the other’s notarized consent on form DS-3053.
