🌐 Go to the official portalin.gov ↗The $10.00 fee is a non-refundable SEARCH fee set by state law (IC 16-37-1-11) and includes one certified copy if the record is found; additional copies bought at the same time are $4.00 each, and an amendment costs $8.00 more. One search covers the year you give plus two years either side. For faster service take the form to the local health department in the county where the birth happened instead of mailing the state — and for births before October 1907 you must, because only that county holds them. Not to this office.
