The years this office can serve. It issues birth records from 1930 to the present. Its page does not say whether that covers events in other Florida counties; older records come from the Bureau of Vital Statistics in Jacksonville.
Who may have one. § 382.025(1) gives a birth certificate to the person named if of legal age, a parent, guardian or legal representative, or — once the registrant has died — a spouse, child, grandchild or sibling of legal age. A birth record more than 125 years old is public.
Ordering from the state Bureau of Vital Statistics costs $9.00 for the first computer certification and $4.00 for each additional copy ordered at the same time; county health department fees vary, so ask before you go. Mail the form to the Bureau in Jacksonville, walk it in there, or take it to a local county health department — which issues Florida birth records from 1917 onward — but not to this office. Include a copy of your photo ID and state your relationship to the person named.
