The years this office can serve. It issues births registered anywhere in Florida from 1930 to the present. Anything earlier comes from the Department of Health's Bureau of Vital Statistics in Jacksonville, which holds the state's older records — so check the year before you make the trip.
Who may have one. § 382.025(1) gives a birth certificate to the person named if of legal age, a parent, guardian or legal representative, or — once the registrant has died — a spouse, child, grandchild or sibling of legal age. A birth record more than 125 years old is public.
Ordering from the state Bureau of Vital Statistics costs $9.00 for the first computer certification and $4.00 for each additional copy ordered at the same time; county health department fees vary, so ask before you go. Mail the form to the Bureau in Jacksonville, walk it in there, or take it to a local county health department — which issues Florida birth records from 1917 onward — but not to this office. Include a copy of your photo ID and state your relationship to the person named.
