Mail the form with a copy of your photo ID and a check or money order to ADH Vital Records in Little Rock, or take it there in person. The Department of Health says it offers “vital records services in all local health units”, but its walk-in instructions name only the Little Rock office, so call your Local Health Unit before going. The County Clerk does not hold death records.
