Death certificates come from Alabama Vital Records — the Center for Health Statistics at the Alabama Department of Public Health — or from a county health department, not the Judge of Probate. Ala. Code § 22-9A-21 puts vital records in the hands of the State Registrar and the custodians the board authorizes.

The $15.00 fee includes one certified copy; additional copies of the same record ordered at the same time are $6.00 each. Take the completed form to any Alabama county health department, or mail it to the Center for Health Statistics in Montgomery — not to this office. Death certificates less than 25 years old are restricted: you must be an immediate family member or show a legal right to the record.
